Insert or full-frame?
Insert where the existing frame and sill are sound, which is cheaper and keeps the exterior trim. Full-frame where they are not. You are told which after the openings are measured, not before.
Condensation between the panes.
The seal has failed. That is a glass unit or a window replacement, and no amount of resealing around the outside will change it.
Do I have to replace all of them at once?
No. Doing the worst elevation first is common, and the rest can follow. The measurements are taken per opening either way.
